Optional Life Insurance
Adequate life insurance coverage is important to you and your family. DPS offers employees a choice of two plan options to purchase additional life insurance for themselves, their spouse or dependent children. Employees may select one or both of these plans to maximize his or her life insurance coverage.
DPS Optional Life Insurance
The following is only a brief overview of your MetLife Optional Life Insurance benefits. Please refer to your blue and white MetLife Booklet for detailed information. If the terms of this plan summary or your certificate booklet differ from your policy, the policy will govern.
New Employee Initial Offer:
As a new employee, you may enroll in the MetLife Optional Life insurance within 60 days of your date of hire. Through the optional life insurance plan, you may purchase additional life insurance for yourself (up to $100,000 without a full statement of health or up to $500,000 with the requirement of a full statement of health). For your eligible dependents, additional coverage -- at reasonable group rates -- is also available. You may cover your spouse (up to $20,000 without a full statement of health or up to $100,000 with the requirement of a full statement of health) of coverage and your dependent children ($2,000 or $5,000 of coverage per child).
Enrollment Requirements after Initial Offer:
Through the optional life insurance plan, you may purchase additional life insurance for yourself (up to $500,000 with the requirement of a full statement of health). For your eligible dependents, additional coverage--at reasonable group rates--is also available. You may cover your spouse ($10,000 up to $100,000 of coverage, with the requirement of a full statement of health) and your dependent children ($2,000 or $5,000 of coverage per child).

An Accelerated Death Benefit is available for employees with a terminal illness whose life expectancy is no longer than six months. A benefit amount of 50% of your life insurance benefit (up to a maximum determined by the insurance company) is payable in a lump sum and determined as of the date certification of your illness is accepted.
You elect your beneficiary(ies) at enrollment. If you need to change beneficiaries, please complete the Beneficiary Designation Form and return it to the DPS Employee Benefit Department, 900 Grant Street, Suite 502, Denver, CO 80203 or Fax it to 720-423-3924. Please note that a change to Optional Life beneficiaries will change the DPS Employer Paid Group Life and AD&D at the same time.
Colorado PERA Optional Life Insurance
PERA offers an optional group decreasing-term life insurance plan administered by Unum called Plan 1. Coverage with Plan 1 is based on age and allows the member to purchase units of coverage to meet their life insurance needs. In addition, the plan provides Accidental Death and Dismemberment (AD&D) benefits, life insurance coverage for spouses and eligible children, and an accelerated benefit option. Unum provides free survivor financial counseling to members, inactive members, and their survivors. The cost to the employee is $7.75 per unit. To enroll, contact UNUM direct toll-free at 1-866-277-1649. Employees have 90 days from their initial date of hire to enroll in the plan or during the PERA open enrollment period April 1 through May 31 without showing evidence of insurability. Employees enrolling outside of these times will need to complete a medical history form, may be asked to take a health exam, and may be denied coverage, if applicable.
