How to complete PACE forms:

If you know the required fields for the Personnel Action you are submitting, please click on the General PACE form provided at the bottom of this page.

If you do not know the required fields, use the appropriate Personnel Action Templates available below to guide you in completing a specific personnel action. Each example provides you with a completed PACE form which shows all the information needed for that specific personnel action. This example is for reference purposes only.

Once you have located the example that best fits your specific personnel action needs, click on the corresponding template. When completing the template be sure to use information specific to your school or department.
