Life and Accidental Death and Dismemberment

Basic Group Term Life Insurance

DPS provides basic life insurance that covers you at all times beginning the day after you complete three months of full-time active employment. DPS pays the full cost of coverage for employees equal to two times your annual salary, up to a maximum of $300,000.

Accidental Death & Dismemberment (AD&D) Insurance

Eligible employees also receive AD&D coverage equal to your basic life benefit. The plan pays a benefit if you die in a covered accident. Reduced benefits are paid if you lose a hand, foot or eyesight in a covered accident.

Optional Life Insurance

New Employee:
Through the optional life insurance plan, you may purchase additional life insurance for yourself (up to $500,000, or 3 times your basic annual eanrings, whichever is less. For your eligible dependents, additional coverage -- at reasonable group rates -- is also available. You may cover your spouse ($10,000 up to $100,000) of coverage and your dependent children ($2,000 or $5,000 of coverage per child).

Open Enrollment:
Through the optional life insurance plan, you may purchase additional life insurance for yourself (up to $500,000 or 3 times your basic annual earnings, whichever is less, with the requirement of a full statement of health). For your eligible dependents, additional coverage--at reasonable group rates--is also available. You may cover your spouse ($10,000 up to $100,000 of coverage, with the requirement of a full statement of health) and your dependent children ($2,000 or $5,000 of coverage per child).

An Accelerated Death Benefit is available for employees with a terminal illness whose life expectancy is no longer than six months. A benefit amount of 50% of your life insurance benefit (up to a maximum determined by the insurance company) is payable in a lump sum and determined as of the date certification of your illness is accepted.
