PERA Optional Benefit Offerings – The merger with PERA has brought employees additional optional benefits. Employees wishing to take advantage of the initial offerings of these benefits need to follow these enrollment instructions:
- The PERA 401(k) offers employees the opportunity to set aside a portion of their salary into a retirement savings account, similar to the 403(b) and the 457(b) that DPS offers. The 401(k) is not tied to the Colorado PERA pension plan in any way. To enroll, just complete the contribution authorization form indicating the percentage of gross pay contribution (not a flat $ amount) and send it to the Benefits Department, fax 720-423-3924. For more information on the 401(k) plan visit PERA’s website.
- Colorado PERA Life Insurance Program is term life/accidental death and dismemberment insurance employees can purchase for themselves, spouse and children. The cost is $6.50 per unit with the limit of four units. To enroll, contact UNUM toll-free at 1-866-277-1649. Visit PERA’s website to view the coverage amounts and program highlights.
- Active, retirees, and inactive PERA members, and their family members are eligible to apply for MetLife Long-Term Care Insurance with reduced underwriting and discounted rates at any time. For more information call 1-888-PERA-LTC or visit www.ltccopera.com.
